This date is used to calculate the value of your pot and you will receive payment 10 to 15 days after this.

Please do not complete this form until you are ready to take your cash lump sum and you have satisfied yourself that no further contributions will be paid by you or on your behalf into this arrangement.

 

Declaration

I wish to take all the benefits under this scheme as a cash lump sum. By doing so, I accept that normally one portion will be free of tax and the remaining portion will be taxed as pension income in the tax year of receipt. The payment from Legal & General will be made with tax deducted.

If I want to take a full cash lump sum, I confirm that:

  • I have read any personalised risk warnings, and I am happy to proceed, and
  • I understand that if I was entitled to protected tax free cash that this would be lost under this option and restricted to 25% of pension pot value, and
  • I have not received a pension credit from a former pension in payment, and
  • I acknowledge that my annual allowance will be reduced, and
  • I undertake to inform all pension providers of my reduced annual allowance within 91 days of receipt of this payment.

However, if I want to take a small pot payment, I only confirm that:

  • My pension pot value under this and any related scheme does not exceed £10,000.00.
  • My payment will extinguish my entitlement to all benefits under the scheme.
  • I have read any personalised risk warnings, and I am happy to proceed.

LUMP SUM ALLOWANCE
I confirm that the amount of tax-free cash shown on the quote together with any tax-free cash sums I have taken from any other pension scheme to date does not exceed my Lump Sum Allowance. If this is not the case, I understand that Legal & General will need to ask me for further information. I will need to contact Legal & General before signing the form.

REINVESTMENT OF TAX-FREE CASH INTO ANOTHER PENSION SCHEME
I do not currently intend, as a result of receiving a tax-free lump sum, to significantly increase contributions to one or more registered pension schemes by an amount equal to more than 30%, or £7,500 if lower, of my lump sum. (This is known as website for more information). If I do recycle my tax-free cash sum, I will inform Legal & General within 30 days as required by HMRC regulations.

I confirm that to the best of my knowledge all of the information provided to you is full and accurate. I agree that this information will be used to determine the benefits to be paid. I agree that any payment(s) made under these instructions are in full discharge of all your liability under this/these arrangements. By making this declaration, you do not waive your right to bring a claim for any other act or omission by Legal & General.

By signing this declaration, I confirm that I have read and understood the 'Important information about your illustration' section and can confirm that I will be retiring due to ill health at the settlement date.
