*You can contribute more than 8%; however the Company will not contribute more than 14%. If you wish to make an Employee contribution in excess of 8%, please select 'Over 8%' and input this percentage in the box that appears.

Additional Voluntary Contribution

(This will be on top of your chosen contribution rate each month)

You can pay an Additional Voluntary Contribution AVC) as a fixed monetary monthly amount. Please complete the box below if you wish to pay AVCs in addition to your chosen contribution rate.

Nomination of beneficiary

It’s important to fill in a Beneficiary Form when you join the Kingfisher Pension Scheme and keep it up to date so that the Trustee knows who you would like the lump sum benefit to be paid to in the event of your death.

I authorise the deduction of contributions from my salary and I understand AVCs will not be eligible for matching contributions from the Company.

Declaration

I understand my contributions will be deducted at the first available payroll cycle.

I understand that my retirement savings will be invested in the default fund, Lifestyle Cash Target and my Selected Retirement Age will be my anticipated State Pension Age. If I am aged over my State Pension Age, it will be 1 year from my next birthday.

I understand I can change my investment or Selected Retirement Age at any time and will refer to the Scheme’s Member Guide for further information.

I understand that I will continue to be an active member of the Scheme until I choose to cease my membership or leave the Company.

I understand once I have been a member of the Scheme for three months, my contribution will be made via SMART Pensions and will refer to the SMART Pension Guide for further information.

I confirm I personally submitted this notice to join the Kingfisher Pension Scheme, a workplace pension scheme.

Your personal data is kept secure and only used for purposes relating to the Kingfisher Pension Scheme. Information may be disclosed for those purposes to the Company, the Trustee’s advisers, the administrators of the Scheme or insurance companies. Mobile phone numbers and email addresses may be used should we need to contact you in relation to your membership or for future communications regarding the Scheme. If you do not wish for us to contact you by email or telephone please do not complete those sections.
